Are you retiring?  Changing jobs or starting your own company?  If you have been an active employee for a company that provided you with the Best Doctors Elite Diagnostic Imaging Service, you can personally take the service with you.  Regardless of where you might be going, or what you might be doing, this individual plan can be yours for $19 / month (+ GST).

A few things you should know:

Making the service your very own is a simple process and will only take a few minutes to complete.  Here are some things you should know:

• The "Individual Plan" option of Best Doctors Elite Diagnostic Imaging Service is only available to people coming from a company that is actively enrolled in the plan.
• Even though we use the words "Individual Plan", one plan and one price covers you and, if applicable, your dependents.
• There is no age or heath restriction for creating an individual plan. If you maintain paid coverage, it is yours for life!
• You have 90 days from when leaving your previous employer to take advantage of this unique option.
• Paid coverage must be continuous from when leaving your previous employer.  For example, if you wait 90 days before signing up, your first payment will need to cover 3 X $19 = $57 (+ GST).
• Monthly payments for the individual plan are done exclusively by "Automatic Withdraw" on any Canadian bank account.
